Support at Home Provider Levels 1–8: All 8 Levels Explained
As the leading model for in-home aged care Australia-wide, Support at Home is Australia’s new in-home aged care program. It replaced Home Care Packages and the Short-Term Restorative Care Programme on 1 November 2025.
Instead of four package levels, the new system has eight ongoing classification levels, plus two short-term pathways, designed to fund care more precisely against each person's assessed needs.
Where Ambition fits: we deliver across all 8 ongoing levels of Support at Home, from light, occasional assistance through to maximum-intensity, complex clinical care, plus the Restorative Care Pathway and the End-of-Life Pathway.

Entry-level
Light, occasional support: domestic help, social support, basic everyday assistance.
(02)
Low
Regular help with household tasks, transport, social participation, and light personal care.
(03)
Moderate
Routine personal care, domestic assistance, and some allied-health or nursing input.
(04)
Intermediate
Daily personal care, regular nursing oversight, allied health, and respite for family carers.
(05)
Substantial
Multiple visits per day, complex personal care, nursing, dementia support, equipment and modifications.
(06)
High
Intensive daily support: complex clinical needs, dementia or behavioural support, in-home rehab.
(07)
Very high
Extended hours of care daily, advanced clinical nursing, complex care coordination, end-of-life pathways.

Maximum
Up to 24/7 support with high-acuity clinical needs: complex dementia, palliative or end-of-life care, advanced nursing. Funding up to ~$78,106 per year.

At Ambition Health Group, we provide a range of My Aged Care funded services designed to help older Australians live comfortably and safely in their homes.

Respectful help with bathing, dressing, grooming, toileting and mobility, every day, on your terms, with dignity.
- Community Nursing & Clinical Care
Qualified nurses for medication management, wound care, chronic disease support, catheter and continence care, and ongoing health monitoring at home.

Cleaning, laundry, dishes, meal prep and the everyday tasks that keep your home safe and your stress low.
- Dementia Care
Specialist, person-centred dementia support, from early-stage assistance through to advanced behavioural and clinical needs. Calm, familiar, consistent carers.
- 24-Hour & Live-In Care
Around-the-clock support for high-acuity needs, with continuity of staff and clinical oversight.
- After-Hospital Care
Short-term, intensive support to bridge the gap home, with wound care, medication oversight, mobility support and daily living help to prevent readmission.
- Palliative & End-of-Life Care
Compassionate, clinically-led care at home for participants on the End-of-Life Pathway, with comfort, dignity, pain management and family support.
- Allied Health & Restorative Therapy
Physiotherapy, occupational therapy, podiatry and fall-prevention programs, focused on rebuilding independence.
- Transport, Social & Community Support
Help to attend appointments, run errands, stay connected and remain part of community life.
NDIS Accommodation & Living Supports
We provide a range of NDIS-funded accommodation and living supports designed to give you the comfort, safety, and independence you deserve.
Supported Independent Living (SIL)
Our SIL services help you live more independently while receiving the day-to-day support you need. From daily tasks to personal care, we ensure you have the right balance of freedom and assistance in a supportive environment.
Specialist Disability Accommodation (SDA)
If you need a temporary living arrangement while waiting for long-term housing or modifications, our MTA services give you a stable and supportive home for up to 90 days, ensuring continuity of care and peace of mind.
Short-Term Accommodation (STA)
Whether it’s for respite care, trying a new living arrangement, or short-term support, our STA services provide a safe and comfortable place for you to stay while your regular carers take a break or until you transition to longer-term living.

Many older Australians qualify for more than one program, including DVA, NDIS for those who joined before 65, or private cover. The same Ambition team can coordinate everything together. One provider. One care plan. No gaps.

When did Support at Home start, and what does it replace?
Support at Home commenced on 1 November 2025, replacing both Home Care Packages and the Short-Term Restorative Care Programme. The Commonwealth Home Support Programme (CHSP) is expected to transition into Support at Home from no earlier than 1 July 2027.
What are the 8 levels of Support at Home?
There are 8 ongoing classification levels, from Level 1 (light, occasional support) to Level 8 (maximum-intensity care up to around $78,106 per year). Your level is set by My Aged Care based on an assessment of your needs. We deliver across all 8 levels, plus the Restorative Care Pathway and End-of-Life Pathway.
Can I still get a Home Care Package, or has it ended?
Existing Home Care Packages have transitioned to the new Support at Home program with a ‘no worse off’ commitment, where your funding level aligns to one of the 8 new classifications and unspent funds carry over. New applications now go through My Aged Care under Support at Home.
Can I self-manage my Support at Home funding?
Yes, self-management options remain under the new program. With Ambition, you can direct your providers, schedule and budget while we handle compliance, care plan oversight and clinical safeguards.
What is care management and do I have to pay for it?
Care management is a defined service under Support at Home, where your care manager assesses, plans, coordinates providers and reviews your care. A portion of your quarterly budget funds it; you don’t pay separately on top.
Will I still pay an income-tested fee?
The old income-tested care fee and basic daily fee have been replaced by a single ‘participant contribution’ based on means and aligned to the service category. Clinical care has no participant contribution; Independence and Everyday Living categories do. We help you understand what you’ll actually pay.
What if my care needs increase? Can I upgrade my level?
Yes. If your needs have changed since your last assessment, we can help arrange a reassessment through My Aged Care to move to a higher classification. Our clinical team can document the change and prepare the supporting evidence.
How quickly can services start after a hospital discharge?
For urgent hospital discharges, we can usually start care the same day or the next business day. Our dedicated discharge pathway is designed specifically to clear hospital bed delays for clinically-stable older patients waiting on community care.
