Do participants get funded for Support Coordination every year?

Support Coordination funding is determined by the NDIS based on your individual needs and circumstances. Not all participants receive Support Coordination funding, and the amount can vary from plan to plan. During your plan review, your need for Support Coordination will be reassessed. If you’ve made significant progress toward your goals and developed skills to manage your plan independently, your funding for Support Coordination might be reduced or removed in subsequent plans.

The number of Support Coordination hours funded varies widely depending on individual circumstances. Typically, participants might receive between 20 and 100 hours per year, but those with complex needs or in crisis situations may receive more. Your Support Coordinator will help you use your allocated hours efficiently throughout your plan period, with more intensive support often needed at the beginning of a new plan and during significant life transitions.

Yes, organisations can provide both Support Coordination and other direct supports, but there are strict guidelines to manage potential conflicts of interest. At Ambition Health Group, we have robust policies in place to ensure that our Support Coordinators provide unbiased advice and respect your right to choose your providers freely. We will always present you with multiple options for service providers and respect your choice, whether or not it includes our own services.

While there is no mandatory qualification for Support Coordinators, most have relevant qualifications in disability, social work, community services, or allied health. At Ambition Health Group, our Support Coordinators have a minimum of a Certificate IV in Disability or equivalent, plus extensive experience in the disability sector. Our Specialist Support Coordinators have additional qualifications such as social work, occupational therapy, or psychology degrees, along with specialised experience working with complex needs.

In emergency situations, your Support Coordinator can help by quickly connecting you with appropriate crisis services, temporarily adjusting your supports to address the situation, and liaising with the NDIS for emergency funding if needed. At Ambition Health Group, we provide our clients with emergency contact details and have protocols in place to respond promptly to urgent situations. We also work with you to develop crisis plans in advance, so everyone knows what to do if an emergency occurs.

You have the right to change your Support Coordinator at any time. To switch to Ambition Health Group, simply contact us and we’ll guide you through the process. This typically involves ending your service agreement with your current provider (usually requiring 2-4 weeks’ notice) and setting up a new agreement with us. We’ll help you ensure a smooth transition, including obtaining necessary information from your previous coordinator and establishing relationships with your existing service providers.
