Ambition Health Group provides Social and Community Connection Support that helps people get out, join in and stay engaged, whether that’s an older person attending a local club, an NDIS participant building friendships, or someone rebuilding their life after injury.
It’s about belonging, not just activities. Our support workers don’t just take people to events. They help them feel comfortable, included and confident, at their own pace.

Connection Support Across Every Funding Program
Whatever funds your care, staying socially connected can be part of it:
- Aged Care / Support at Home: for older Australians, we provide Social Support & Community Connection as an Independence service under Support at Home. Our In-home Social Support for Seniors and Aged Care Social Support Services Australia help seniors attend clubs, reconnect and reduce isolation, with My Aged Care Social Support to help you access it.
- NDIS: NDIS Social and Community Participation (and Increased Social and Community Participation NDIS) helps participants build skills, friendships and independence funded under Core Supports or Capacity Building.
- TAC & iCare: Community Reintegration support to help people return to social, recreational and community life after a serious injury.
- DVA: Veteran Social Connection Support for eligible veterans, easing isolation and keeping them engaged in their community.
- Private: flexible, fee-for-service companionship and community support, no funding program required.

Who can access social and community support?
Older Australians (through Support at Home / My Aged Care), NDIS participants, people recovering from a serious injury (TAC, iCare), eligible veterans (DVA), and private clients. We’ll help you understand how your funding can cover it.
Which areas do you cover?
We provide dedicated local support hubs across VIC, NSW, QLD and TAS, with national expansion underway. Available activities and coverage vary by location. Contact us with your suburb and we’ll confirm what’s on offer.
What activities can I take part in?
Sports and fitness, arts and crafts, cooking groups, community outings, volunteering, social clubs, travel training and skill-building, plus simple companionship at home. We tailor everything to your interests.
How is it funded?
For older Australians, it’s part of the Independence services under Support at Home. For NDIS participants, it’s funded under Core Supports or Capacity Building (Increased Social and Community Participation). It’s also supported under TAC, iCare and DVA, or privately.
Can I choose which activities I do?
Always. We take a person-centred approach, where you choose the activities that match your interests and goals, and we provide the support to take part comfortably and confidently.
Can you help someone who can't easily leave home?
Yes. As well as community outings, we provide in-home companionship and social support, including a friendly visit and conversation for those who are more housebound, easing isolation right where they are.
Can you help with transport to activities?
