How It's Funded
For older Australians, transition support is delivered under the Support at Home Program Australia, including the Restorative Care Pathway and our Aged Care Nursing Services Australia. NDIS participants and people under TAC, WorkSafe and DVA are also supported, and private care can start immediately if you need help before funding is confirmed. Whatever your funding pathway, our goal is the same: reliable care at home after discharge from hospital.
