What Is Support at Home?
Support at Home is the Australian Government’s in-home aged care program. It began on 1 November 2025 and replaced Home Care Packages. If you’ve been searching for a “Home Care Package,” Support at Home is what you’re looking for now.

Clinical Supports. Our heritage: nursing-led care
This is where our nursing roots set us apart. Clinical care is delivered with Registered Nurse oversight, making the difference between a basic support provider and one that can safely manage real health needs at home.
- Nursing Care at Home
Our home care nurses Australia wide deliver Registered Nurse led clinical care in your own home, including: wound care and management, medication support and administration, chronic disease and diabetes support, continence support, and health monitoring and post hospital nursing. - Allied Health
Coordinated therapies to keep you strong, mobile, and independent: physiotherapy and exercise physiology, occupational therapy (OT), podiatry, speech pathology, and dietitian and nutrition support. - Palliative & End of Life Care at Home
Compassionate, clinically led comfort care for older people who wish to remain at home. The Support at Home End of Life Pathway supports people with a life expectancy of around three months, complementing state and territory palliative care services, with dignity, symptom monitoring, nursing support and support for the whole family.

Support at Home Classifications 1–8: Explained Simply
Instead of the old four package levels, Support at Home has eight ongoing classifications based on how much care you need. Here’s what each group generally covers:

Two short-term pathways too: the Restorative Care Pathway (short-term allied health and nursing to help you regain or maintain function, ideal after a hospital stay) and the End-of-Life Pathway (higher-intensity support for comfort and dignity at home).

How to Access Home Care Services and What to Do Next
There are a few ways to access in-home care, whether you need aged care support, disability home care or home care for disabled family members. - Home Care Package transition: if you already have a package, we support your move to Support at Home with the “no worse off” commitment.
- Commonwealth Home Support Programme (CHSP): entry-level support for lower needs (transitioning to Support at Home from no earlier than mid-2027).

Is Support at Home the same as a Home Care Package?

What kinds of services can I get?
Support at Home is organised into three categories, and we deliver across all of them: Clinical Supports (nursing, allied health, palliative care), Independence Services (personal care, respite, transport, social support) and Everyday Living (domestic help, meals, gardening and home maintenance), plus assistive technology and home modifications.
What are the Support at Home classifications?
There are eight ongoing classifications based on your assessed needs, from lower-level support (1–2) through to very high needs and complex clinical care (7–8). My Aged Care assigns your classification. We support clients across all eight.
Can you help with equipment or changes to the home?
Yes. Through the Support at Home assistive technology and home modifications (AT-HM) funding, we help you access and coordinate things like grab rails, ramps, mobility aids and bathroom modifications, usually starting with an occupational therapy assessment.
Do you provide nursing, or just basic home help?
Both. We’re nursing-led, with Registered Nurse oversight covering wound care, medication support, chronic disease management, palliative care and post-hospital recovery, alongside personal care, domestic help, allied health and home maintenance. That means we can keep supporting your loved one as needs increase.
Can you help my parent come home from hospital safely?
Yes. Our Hospital-to-Home team provides urgent assessments, short-notice support and the Restorative Care Pathway, including medication, wound care, personal care and domestic help, so older Australians can return home safely rather than staying in hospital longer than needed.
Can we arrange care privately, without waiting for funding?
Yes. We offer private, family-funded in-home aged care that can start without waiting for a My Aged Care assessment, often used as a bridge while government funding is arranged.
What is care management?
Care management is a defined, funded part of Support at Home. Your dedicated care partner coordinates your services, reviews your plan and adjusts care as needs change, funded from your quarterly budget rather than billed separately.
