NDIS Support Coordination and Specialist (Level 3) Coordination, Across Australia
Expert navigators for the most complex NDIS journeys, person-centred, clinically experienced, and nationwide.
The NDIS can be overwhelming, and the more complex your situation, the harder it is to navigate alone. As a registered NDIS support coordination provider, Ambition Health Group delivers person-centred support coordination across Australia, including NDIS Specialist Support Coordination for participants facing the most challenging circumstances. We don’t just organise appointments. We untangle complexity, reduce risk, and build a stable support system around you.
Support coordination helps NDIS participants understand their plan, connect with services, and effectively use funding to achieve personal goals and greater independence.
Support coordination is an NDIS funded service designed to help individuals understand and use their NDIS plan effectively. Support coordinators guide participants in accessing community, mainstream, and government services that align with their goals and lifestyle.
Support coordinators also help participants navigate their NDIS plan, connect with reliable service providers, and build the skills and confidence needed to manage their supports independently over time.
The Three Levels of Support Coordination
- Support Connection
- Support Coordination
The core service: building your understanding and capacity to use your plan, connecting you with quality providers, and coordinating a mix of supports so everything works together. Every support coordinator NDIS funds at Level 2 works to build your long-term capacity, not just manage your plan short-term.
- Specialist Support Coordination (Level 3)
Supported Independent Living (SIL)
The support that helps you live as independently as possible.
Specialist Disability Accommodation (SDA)
Specialist Disability Accommodation: long-term, purpose-built housing for participants with extreme functional impairment or very high support needs.
Medium-Term Accommodation (MTA)
Transitional housing for up to 90 days while you wait for a long-term home.
Short-Term Accommodation (STA)
Short respite stays.
Individualised Living Options (ILO)
What's the difference between Support Coordination and Specialist Support Coordination?
Standard Support Coordination (Level 2) helps you understand and use your plan, connect with providers and coordinate supports. Specialist Support Coordination (Level 3) is for complex, high-risk situations and is delivered by coordinators with clinical, allied health or social work backgrounds who can manage crisis, risk and significant barriers. We’re a Registered Provider for both.
What is Psychosocial Recovery Coaching, and how is it different?
Recovery Coaching is a separate NDIS support for participants with psychosocial (mental health) disability. Unlike support coordination, which focuses on connecting and coordinating supports, a recovery coach works on building recovery skills and capacity, responsive to the episodic nature of mental health. Some participants have both.
Do participants get funded for support coordination every year?
It’s determined by the NDIS based on your individual needs. Not everyone receives support coordination funding, and the amount can vary between plans. At your plan reassessment, your need is reviewed. If you’ve built the skills to manage your plan more independently, funding may reduce in future plans.
How many hours of support coordination are typically funded?
It varies widely based on individual circumstances and complexity. Participants with complex needs or in crisis generally receive more. Your coordinator helps you use your allocated hours efficiently across the plan period, often more intensively at the start of a new plan and during major life transitions.
Can one organisation provide both coordination and direct supports?
Yes, but it must be managed transparently to avoid conflict of interest. We always present you with multiple provider options and respect your choice freely, whether or not it includes our own services. Your independence in choosing providers is protected.
What qualifications do your support coordinators hold?
There’s no single mandatory qualification, but our coordinators hold a minimum Certificate IV in Disability or equivalent plus extensive sector experience. Our Specialist (Level 3) coordinators hold additional qualifications such as social work, occupational therapy or psychology, with experience in complex needs.
How do support coordinators help in an emergency?
Your coordinator can quickly connect you with crisis services, temporarily adjust your supports, and liaise with the NDIS for emergency funding if needed. We provide emergency contact details, have response protocols in place, and develop crisis plans with you in advance so everyone knows what to do.
How do I change my support coordinator?
You can change at any time. To switch to Ambition Health Group, just contact us. We’ll guide you through ending your current service agreement (usually two to four weeks’ notice) and setting up a new one, including obtaining your information from your previous coordinator and re-establishing provider relationships for a smooth transition.
