Applying for a Home Care Package can feel overwhelming, but at Ambition Health Group, we’re here to guide you through each step of the process. Follow these five key steps to access the care and support you need.
Register with My Aged Care
To begin, you must register with My Aged Care, the government’s central portal for aged care services. You can do this by:

Why Register Early?
Registering early ensures you are placed in the system and can receive timely support when you need it. Your registration remains active, so you can reapply or update your circumstances anytime.
💡 Tip: You can appoint a family member or friend as your My Aged Care representative, allowing them to act on your behalf and assist with managing your care.
Get Assessed
After registering, you will be referred for an Aged Care Assessment Team (ACAT) assessment to determine your eligibility and level of care. This assessment typically includes:
- A home visit from a trained assessor
- A discussion about your daily living needs, health conditions, and lifestyle goals
- Recommendations on whether you qualify for a Home Care Package and at what level (1-4)
Following the assessment, you will receive a letter confirming your eligibility and care level.

Once approved, you may be placed on a national waiting list. During this time, you can:

Choose Your Home Care Provider
When your Home Care Package is allocated, you will receive a letter from My Aged Care. You then have 56 days to select a government-approved provider like Ambition Health Group to manage your care.
